Medical Coordinator

Qualifications:

  • Must be 18 years of age or older
  • Must complete physical examination and criminal record check (Act 33 clearances)
  • Must have medical experience
  • Must have experience with people with intellectual disabilities

Mattern House is committed to providing professional care to the individuals we serve.  Medical coordinators are responsible for facilitating care coordination services for all of our individuals.  Each medical coordinator is assigned a caseload for whom they are responsible.  It is Mattern House policy that our medical coordinators ensure that our individuals receive timely, high quality and efficient care and support services through the development of a multi-disciplinary care plan, self-management goals, and referrals to external resources.  All services provided by the medical coordinator must fulfill and comply with all current state and federal regulations.

Each medical coordinator is responsible for completing the following duties to Mattern House standards:

  • Coordinate with program specialist to develop new medical files upon arrival of new admissions
  • Identify and link individuals with community resources to facilitate referrals and respond to medical needs
  • Schedule and coordinate all medical and dental services, both as required and on an as-needed basis
  • Fulfill all medical requirements as listed in the Individual Health section of the Chapter 6400 regulations
  • Track and support individuals when they obtain outside services such as Emergency Departments, Hospitals, and any other health care facility
  • Provide timely and up-to-date information to families/guardians regarding all medical needs, appointments, test results, and care plans
  • Ensure timely follow-up to medical professionals regarding test results or any appointment that may result in a change of care plan for the individual
  • Obtain doctor’s orders for any medical needs that require a change in care
  • Arrange appropriate care and supplies at home for each individual needing care and train staff accordingly
  • Coordinate any pertinent information to each individual’s day program when applicable
  • Scan all medical documentation to each individual’s Mattern House team member upon return from all medical appointments
  • Ensure that each individual’s support coordinator is up to date with all current medical information
  • Provide up-to-date contact information with all medical providers including but not limited to work, cellphone, and medical office number.
  • Respond to all medical professionals in a timely manner
  • Maintain availability as an emergency contact for any emergency situation unless alternate arrangements have been made such as a secondary emergency contact or if an on-call contact has been assigned
  • Assist individuals with the process of determining eligibility, obtaining, and maintaining free or low cost care and/or pharmaceuticals
  • Collaborate with all members of the individual’s team to create care plans for all individuals including individuals who require high risk care
  • Submit all prescriptions to the appropriate pharmacy and ensure timely delivery to the homes
  • Educate all individuals on their plan of care and progress towards their goals
  • Establish and maintain positive, professional relationships with all medical service providers
  • Understand and effectively communicate HIPPA compliance, company policy compliance, and individual confidentiality
  • Maintain communication with the medical coordinator supervisor on any updates, changes, or questions about care plans
  • Maintain communication with LPN/RN and utilize as a reference for any medical questions or concerns
