Intellectual disability services and developmental disability services to assist people in achieving the life they want!
Mattern House, Inc. designs its intellectual disability services and developmental disability services program around each individual’s needs, wants, desires, and capabilities. Every person that we support will be part of an interdisciplinary team that meets to review and monitor the individual’s situation. This team will include:
The individual, the individual's family, legal guardian, surrogate or advocate
The support coordinator
Providers of service, specifially direct care staff
The common law employer or managing employer, if the individual has chosen to self-direct
Any other persons designated by the individual
This team assures the health, safety, rights, and happiness of the individual through the Individual Support Plan (ISP) process. This process, led by the Supports Coordinator (SC), is the most critical activity to help people envision a good life and to develop strategies to achieve the life they want. It is a process to help people explore the experiences, opportunities, and resources available to them through family, friends, and the community. It is also the process to identify which services can enhance those resources and opportunities. The SC should assist the individual and family to understand and participate in the ISP process. This includes thinking about relationships that are important to the individual; activities that are enjoyable and important to the individual; what kinds of growth experiences the individual would like to explore; what kind of job the individual would like; whether there are any health or safety risks that must be planned for; what the immediate needs are, as well as the needs the individual or family anticipates for the future, and what types of services would assist the individual and family in achieving the quality of life they hope to have.
Each ISP also contains additional information such as contacts, insurance, individual preferences, medical information, health and safety, functional and financial information. All staff that work with the individual are trained on the current ISP and subsequent updates. Direct support supervisors and direct support staff use daily notes to document the individual’s activities. The program specialist reviews these daily notes to ensure that the outcomes set in place in the ISP are being implemented and fulfilled. The team meets periodically to review the ISP and to update and make any changes. The county administrative entity monitors the daily service notes for the individuals to ensure individuals are receiving the services determined in the ISP. Licensing representatives from the Office of Developmental Programs review the residential records to determine if residential programming is being carried out as outlined in the 6100 and 6400 regulations that are put in place for Residential Providers.
Our Intellectual Disability Services
Financial Management
Every person who lives at Mattern House receives support from staff to help them manage their finances according to the regulations that are put in place by the State of Pennsylvania. All staff who work with the individuals are trained on proper procedures when dealing with money for the people who live at Mattern House. If the individual does not have a Representative Payee, then Mattern House will consider applying for that role.
Transportation
Every Mattern House home has a vehicle available to transport the individuals to day programs, community outings, appointments, shopping, work, etc. There are also non-ambulatory vans available for those that need to utilize a wheelchair. Our intellectual disability services and developmental disability services staff are trained to properly use tie-downs for the wheelchair vans.
Medical Support
Our intellectual disability services staff are trained on medication administration as mandated by the state of Pennsylvania to aid in giving medications to the people that we support. We also have a medical department that reviews recommendations from the doctor for each person we support and implements those recommendations. The medical staff also take the individuals to doctor’s appointments for routine check-ups, yearly physicals, immunizations, dental appointments, and any other medical appointment that they would need to go to. We encourage families to be a part of supporting the individual at their appointments, if they prefer. The team members for each person we support receive documentation of each appointment.
