Applying to live at Mattern House
Mattern House, Inc. does not exclude any person from admission solely on the basis of race, religion, creed, sex, age, sexual orientation, sexual identification, or national origin.
There are several different factors involved in determining if a person is eligible to live at Mattern House. If an individual lives in Blair County and does not receive Intellectual Disability services through a support coordination agency, then we refer them to our Administrative Entity:

SASMG is responsible for coordinating and assuring quality of Intellectual Disability services throughout Blair County. We ask the individual/family to reach out to SASMG to get direction on where to begin the process to obtain services.
If the individual already receives Intellectual Disability services, then we ask them to talk to their Support Coordinator about applying for Residential Services through the SASMG Request for Proposal Database. Mattern House would then respond to the RFP and our committee would review information obtained from the Support Coordinator and set up a Meet and Greet meeting with the Individual and the family/team to see if we have a suitable vacancy for the person looking for a home.
Once the determination is made that we have a suitable vacancy to meet the needs of the individual, then a tour would be set up for the person along with their family and support coordinator to see the home and decide if they want to live there. We would also set up for the individual to have scheduled
visits with the other people who live in the home so that they can get a sense of who they will be living with and give them a chance to see if they will like the home they will be moving to. If the individual and the team wish to move forward, then all the pertinent paperwork would need to be completed
prior to the individual moving in and a move-in date would be set.
Mattern House Application Steps
Prior to admission, the applicant agency must complete an individual admission form and provide copies of medical and psychological examinations by competent personnel. We have additional
documentation that will need to be completed before the move-in date as well. All of this paperwork will be submitted to the Program Office. We have provided some of our forms below:
